World Health Day - World Record "Hokey Pokey Challenge"

Post Challenge Verification Statement

In order to qualify for the Guinness World Record challenge, following the event - each school will be required to provide verification of their participation in the form of a written statement (sample below) and provide a list of all participants for the official submission to Guinness World Records Limited.

Post Challenge Verification Statements should be prepared on your schools letterhead and MUST be signed by the school's Principal or Vice Principal, and the school contact identified at the time of pre-registration. The letter should include the following information:

• Location of the record attempt (Country, Province, City and School)
• Confirmation of the Number of participants
• Date and time of the record attempt
• Details of the record attempt
• List of all participants (names will be kept confidential and will NOT be published)

Please accept this letter as confirmation that on Tuesday, April 9, 2002 at exactly TIME (local start time including time zone) XXXX number of individuals of SCHOOL NAME, in CITY, PROVINCE, COUNTRY participated in the activ8 the Nation's Schools "Hokey Pokey" Challenge.

The challenge required participants to perform verses right arm, left arm, right leg, left leg and whole self in for a minimum of eight minutes.
